10/25/2023 EnchantaVaughn Approved Behavior: QMHPA greeted Ms. Broadnax upon arrival to the home setting. Upon arrival the home, Ms. Broadnax appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her morning. Ms. Broadnax discussed with QMHPA her morning was well. Intervention: QMHPA began the session by inquiring about Ms. Broadnaxâs current thoughts, feelings, and emotions. QMHPA reviewed with Ms. Broadnax the importance of staying focused and organized when budgeting. QMHPA prompted Ms. Broadnax to use her âjournalingâ skills to provide a visual of what her budget was and what she needed to obtain before she received her monthly assistance for next month. QMHPA encouraged Ms. Broadnax to use her positive thinking skills. QMHPA supports improving/implementing daily communication skills such as focusing and listening and responding effectively. QMHPA educated Ms. Broadnax on how this activity and communication can help resolve conflicts and avoid negative confrontations. QMHPA engaged Ms. Broadnax in a one on one intervention in a community setting to increase listening skills and focusing skills. Response: Ms. Broadnax appeared to be receptive to QMHPAsâ interventions and services that were provided to her during the session. During the session, Ms. Broadnax admitted that stress has caused her to sometimes make irrational decisions which cause her to forget and then in an angry or depressed mood. Ms. Broadnax shared that after a couple days she gets an attitude and just say forget it. Ms. Broadnax shared that she will begin taking some time for herself. Ms. Broadnax was able to identify some healthy coping skill that reduces stress. Ms. Broadnax shared that she likes to walk but not exercise. Ms. Broadnax shared that 90% of the time she is not able to cope with her impulses and behaviors triggered by anxiety and depression. Ms. Broadnax expressed understanding the difference of listening to understand verses listening to reply. Ms. Broadnax had to be redirected so that she could identify effective communicating and decision making so that she can meet the needs of her family. Plan: QMHPA will continue to teach Ms. Broadnax social skills and communication skills through modeling and role-playing to improve her relationships with others 3-4 times (Weekly).
